Adult Day Program Nurse, RN or LPN

Per diem

Provide clinical expertise to the Adult Day Program

Education:
Bachelor's degree in nursing with a certification in gerontology or rehabilitation desirable

Experience:
* Minimum two years of community health or related experience, or a combination of education and experience from which comparable knowledge and skills are acquired
* Broad base of clinical knowledge and skill relevant to the health care of the elderly

License Requirements:
Current licensure or compact licensure recognized by the State of Vermont required

Performance Standards:
* Check that excellent systems are in place for tracking what clinical documentation is due in the Adult Day chart, what remains incomplete, and what has been completed
* Check that established clinical systems are being correctly used as designed
* Work with manager /director as needed to help redesign medication order process in Adult Day
* Spot check that charts are accurate and complete in preparation for regular audits (at least once every 2 months)
* Be a senior clinical resource to clinical staff, listen to their client questions/concerns, and help provide solutions when possible, direct them to additional resources when the issues are outside your scope (Manager/Direct of Community Care Connection). In consultation with manager, directly assess clients and make home visits if necessary to assess complex situations. Coordinate clinical services for enrolled clients including maintaining clinical records; conducting periodic clinical evaluations, conferences; contracting families and other involved professionals to review needs and resources
* Provide health education/counseling to participants, families, staff, and public
* Administer medications and provide emergency care
* Prepare all required clinical and administrative documentation related to assigned activities, adhering to all applicable legal regulations, technical requirements of government agencies, and internal .procedures
* Other duties as assigned by supervisor
