Seeking RNs to join our interdisciplinary team providing skilled nursing services to clients in their home. We offer an intensive mentoring program upon hire and ongoing education. We have a variety of schedules available to help you have a work/life balance including: part-time, full-time, and various baylor positions.

The Community Health Nurse, RN will be responsible for the delivery of a full range of skilled nursing care to individual clients in their homes and other community settings. Care is family centered, age specific and culturally appropriate. The Community Health Nurse II (CHN) provides nursing care to clients utilizing the Nursing Process and within the framework of ANA Standards of Practice for Home Health Nursing, and Specialty Specific Standards of Patient Care. (CWOCN, APA, etc.) The CHN II assumes leadership responsibilities, including delegation of work to other employees.

Qualifications

• ADN or higher degree in nursing
• Greater than one year of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired
• Current RN licensure or compact licensure recognized by the State of Vermont required
